Diabetes Self-Management Education/Training/Support Services
The Diabetes Self-Management Education/Training/Support Program (DSME/T/S) offered through St. Mary’s Health and Clearwater Valley Health is aimed at empowering patients to take charge of their health. Currently this is accomplished in a group or one-to-one manner offered by one of our Diabetes Care and Education Specialists.
We work to accomplish your needs! Appointments can be made to be seen in Orofino, Cottonwood, Kamiah or Grangeville. We work to accomplish your needs! The program focuses on assessing your readiness to engage, personal preferences, and providing training/education/support so that you can gain a comfort level and knowledge in dealing with your individual situation.
Major topics addressed include: Nutrition, Physical Activity, Monitoring, Medications, Problem Solving, Risk Reduction and Coping Skills. Based on individual needs and preferences we may also explore how a diagnosis of diabetes impacts pregnancy, sexual health, aging, fall risk, heart disease and stroke, cancer and dementias. Services are available for people of all ages with Type 1, Type 2 and Gestational Diabetes.
Our educators are part of a team that includes Medical Providers (MDs, DOs, NPs, & Pas; whether they are part of our practice or outside providers), Nursing Staff, Community Health Workers, Behavioral Health Specialists, RN Case Managers, Dietitians, Podiatrists, Dentists, Eye Care Specialists and Benefit Counselors. Working together as a team we can provide optimal health oversight and advice.
*Appointments are available during regular business hours and special effort will be made to extend hours for evening or weekend appointments, if necessary, to fit patient schedules.”
