Diabetes Self-Management Education/Training/Support Services

The Diabetes Self-Management Education/Training/Support Program (DSME/T/S) offered through St. Mary’s Health and Clearwater Valley Health are geared to empower patients to take control of their health. Currently this is done in a one-to-one manner offered by one of our Diabetes Educators.

Appointments can be made to be seen in Orofino, Kooskia, Pierce, Cottonwood, Kamiah or Grangeville. The focus is on assessing the patient’s readiness to engage, their preferences, and providing training and education so that they can gain a comfort level and knowledge in dealing with their individual situation.
Major topics addressed include: Nutrition, Physical Activity, Blood Glucose Monitoring, Medications, Problem Solving, Risk Reduction and Coping Skills. Based on individual needs and preferences we may also explore how a diagnosis of diabetes impacts pregnancy, sexual health, aging, fall risk, heart disease and stroke, cancer and dementias. Services are available for Type 1, Type 2 and Gestational Diabetic patients of all ages.

The educators are part of a team that includes Medical Providers (MDs, DOs, NPs, & Pas; whether they are part of our practice or outside providers), Nursing Staff, Community Health Workers, Behavioral Health Specialists, RN Case Managers, Dieticians, Podiatrists, Dentists, Eye Care Specialists and Benefit Counselors. Working together as a team we can provide optimal health oversight and advice.

*Appointments are available during regular business hours and special effort will be made to extend hours for evening or weekend appointments, if necessary, to fit patient schedules.
